Home Health & Hospice Care and Solution-Health have signed a combination agreement after winning approval from the Charitable Trusts Unit of the NH Department of Justice.
The agreement makes HHHC an official member of the SolutionHealth network, which includes Nashua-based Southern New Hampshire Health and Manchester-based Elliot Health System, which joined to form the entity in 2018.
“We are absolutely pleased to formalize our affiliation with Home Health & Hospice Care,” said Brad Kreick, SolutionHealth CEO. “Together, we will meet patients with the care they need, where they need it. We’ll do this collaboratively, to provide the best of our services for healthier communities.”
The agreement allows HHHC to work with Elliot Health System’s VNA of Greater Manchester and Southern New Hampshire to develop a fully integrated, regionally coordinated system of post-acute care. Integration plans have already begun in an effort to support a seamless transition and build the framework for future success.
